The current contract for General-practitioner services in Derby is projected to end on 20/3/2017 and may be available for re-tender.The current contract for General-practitioner services in Derby included carrying out an exercise to identify a caretaker provider for a GP Surgery on a short term APMS Contract. Southern Derbyshire Clinical Commissioning Group (SDCCG) and NHS England (North Midlands) are looking for suitable qualified providers with an ability to deliver a GP Surgery service for up to 12 months (9 months plus 3 month notice period).During the 12 month caretaking arrangement, it is envisaged that the long term future of the service will be determined through public consultation and, depending on the outcome of the consultation, possible re-procurement.The practice has a current list size of approximately 11 000 patients and is provided from 2 locations:The Meadowfields Practice Chellaston.Fellow Lands Way, Chellaston, Derby, DE73 6SW.The Meadowfields Practice AllentonColeman Health Centre, 6 / 13 Coleman Street, Allenton, DE24 8NHThe identified provider will have the option to continue to provide services from these premises.The Commissioners are looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets.The model will need to include provision of the following: Essential Services; All Additional Services; Dementia and Learning Disabilities; Enhanced Services. Opening hours and service delivery from 8:00 to 18:30 Monday to Friday (except statutory bank and public holidays).The full scope of the service can be found in the attached draft specification; it is essential that interested providers must be able to demonstrate an ability to mobilise to service provision within a short time frame. The Caretaking Service is due to Commence 1.1.2016.The Commissioners wish to invite suitably qualified and experienced providers to bid for the provision of this service.
